It stands on a property purchased in 1910 by Adam Chełmoński, near the former miller's cottage. It is a memento of the former settlement of Stare, which is now part of the village of Adamowizna.
The mill standing nearby operated until 1957. The miller's house was designated by Adam Chełmoński as a school.
In the place where the chapel stands, on the road leading to the Chełmoński manor, there used to be a figure of the Virgin Mary on a pedestal made of field stones. In 1962, the chapel was secretly rebuilt by making a cement pedestal and placing a new figure in a glass casing.
