The church, among the oldest in Foligno, stands on the western side of Piazza San Domenico, where there was an 8th century chapel dedicated to the Assumption, Saint Peter and Saint Paul.
There is information that in 1087 the hospital of Santa Maria was annexed to it.
The construction of the Romanesque church took place in the 12th century, the original chapel was incorporated into the left nave.
The façade in bands of white and pink stone is the result of an unfortunate nineteenth-century rearrangement which reused elements of a previous rose window to create a mullioned window, not proportionate to the façade; it is preceded by a small portico, also rebuilt in the 19th century, reusing columns and capitals from the 11th or 12th century.
To the right of the portico there is an aedicule dating back to 1480 with a sketch of a fresco of Sant'Anna attributable to Mezzastris; at the end of the right side is the Romanesque bell tower.
